| To seek treatment services at the Nightmare Treatment
Center, you must take the following steps: 1. Contact Dr. Barry Krakow by e-mail and include a brief statement about the nature of the problem, the communication interface you prefer and your method of payment. 2. Dr. Krakow will then reply by e-mail to offer you potential appointment times. At this point, you will also been given access to the Sleep History** forms, which you can download, complete and fax or mail back to the Center. 3. Once these matters have been attended to, treatment can usually begin within a few weeks to a month's time. Sleep Medicine History Questionnaire **To provide you with better service and more effective care, once you have decided to seek treatment, I will request that you complete an extensive questionnaire that will provide the necessary background to complete your evaluation. This questionnaire can be downloaded for you to complete at your convenience and sent by mail or fax. The form will probably take about 15 minutes to complete. For your information, the following is an outline of the areas explored in the questionnaire: |
I. Chief Complaint - What is the primary complaing you are seeking treatment for? II. History of Present Illness - Questions about the duration and severity of your sleep complaints. III. Sleep Habits - Questions about sleep behaviors: IV. Past Mental Health History - Questions about past and present mental health conditions. V. Past Medical History - Questions about past and present physical health conditions. VI. Review of Systems - Questions about physical and mental symptoms which may be affecting your sleep. VII. Family History - Questions about sleep disorders in your blood relatives. VIII. Social History - Questions about your work, relationships, and education. |
